Johns Hopkins University Community Health Nurse/Neighborhood Nursing Program in Baltimore, Maryland

The Johns Hopkins School of Nursing (JHSON) Center for Community Programs, Innovation, and Scholarship (COMPASS Center) seeks a Community Health Nurse to join the Neighborhood Nursing Program.

Neighborhood Nursing will actively promote primary and secondary prevention services through the registered nurse's capacity to provide health assessments and screenings, vaccinations, health education, and care coordination in the community setting.

Specific Duties & Responsibilities

  • Meet with individuals and families in a variety of community settings including homes, community centers, libraries and other “hub”sites.

  • Conduct a semi structured clinical interview to identify and prioritize areas relating to physical and social well-being using a strength- based framework.

  • Create goal planning based on client identified goals.

  • Deliver nursing component of Neighborhood Nursing in dyad model with Community Health Worker (CHW).

  • Collaborate, communicate and coordinate follow up care with Neighborhood Nursing team.

  • Conduct physical health assessments and SDOH screenings and initiate appropriate referrals based on findings with follow up to ensure closed loop process.

  • Prioritize acute (urgent) and primary/secondary prevention health and social needs.

  • Provide Point of Care (POC) testing (e.g. blood glucose), vaccinations, health education and other activities, in groups and individually.

  • Engage participants in problem solving to identify behavioral and other environmental contributors that impacts their health (physical, emotional and social).

  • Provide mental health and substance disorder psychoeducation in a group or individual setting.

  • Develop and provide health education and training materials for clients and community members.

  • Initiate and assist in developing strong community relationships, attend community meetings and other activities that cultivate trust.

  • Assist Neighborhood Nursing team members and leadership in assessing and evaluating program.

  • Timely data input and documentation in electronic health records and other data management systems.

Technical qualifications or specialized certifications

  • Nursing license

  • Basic Life Support certification (BLS)

Physical Requirements

  • May require lifting and walking stairs.

Minimum Qualifications

  • Individual must be a registered nurse, licensed in the State of Maryland or state where practicing.

Preferred Qualifications

  • Bachelor's Degree in Nursing from an accredited college or university.

  • Two or more years’ experience in community health.

Starting Salary Range: $64,000 - $112,200 Annually ($86,390 targeted; Commensurate with experience)
