Overview The primary duty of this position is to assist and support the collaborative work activities that San Juan County Health Department (SJCHD) and the Preschool Development Project (PDG5) at Utah State University (USU) are developing together in San Juan County, Utah. This position will help to improve connections between families and preschool-age health resources, and it includes opportunities to support community outreach, help in the design and delivery of trainings, facilitate collaborative activities with SJCHD programs focused on children birth to eight years of age and their families, facilitate networking with other Early Childhood Care and Education organizations in San Juan County. This position has no supervisory responsibilities and will report to both the SJCHD and PDG5 designated staff.

Responsibilities

  • Develop and maintain collaborative activities with SJCHD programs focused on children birth to eight and their families.
  • Develop and maintain networking with other Early Childhood Care and Education, birth to eight organizations in San Juan County.
  • Support community outreach, including looking for, finding, and evaluating diverse children and families who may be eligible to use services provided by SJCHD.
  • Deliver county wide training mutually agreed upon by the USU and SJCHD team leadership.
  • Assist on work tasks, as appropriate, to support SJCHD and PDG5 collaboration goals, and coordinate efforts to ensure that quality work is completed on time.
  • Participating in project meetings and project activities as needed.

Qualifications Minimum Qualifications:

  • Community Health Work (CHW) certificate (or equivalent) or committed to complete the CHW program at the beginning of the job.
  • Experience working with rural/frontier populations.
  • Reliable personal transportation.
  • Experience helping people connect to the help and resources they need, such as health, education, and finances.
  • Experience bringing attention to the needs of communities and families.
  • Experience crossing cultural differences, especially those whose voices are not commonly heard. Preferred Qualifications:

  • Experience working with San Juan County communities.

  • Experience with San Juan County agencies and/or coalitions.

  • Experience working with early childhood programs.
  • Experience working with children with disabilities.
  • Experience working with interdisciplinary teams.
