Position Summary:

As an integral member of the Interdisciplinary team in the Weight and Wellness Clinic at Signature Medical Group, the Clinical Social Worker provides psychosocial assessment, short term focused counseling, psycho-education , and community referrals to patients in both the surgical and medical weight loss programs, through individual and group work. This individual is responsible for the peri-operative psychosocial assessment which is designed to screen and identify potential psychosocial risk factors which may impact successful completion of the program. The Clinical Social Worker develops mutually defined goals with the patient in collaboration with the interdisciplinary team to address these potential areas of concern in support of the patient’s weight loss journey. The qualified candidate will have experience in a medical setting as well as expertise in evidence based practices relevant to bariatric social work and cultural competence in care delivery.

Responsibilities:

  • Initial and follow up Psychosocial Assessments to evaluate a patient’s preparedness for medical weight loss interventions

  • Recognizes and diagnoses relevant behavioral health conditions using the DSM-V-TR

  • Collaborates with the providers and other members of the multidisciplinary team to establish measurable healthcare goals and prioritization of the patient’s needs as well as identifying any barriers to care

  • Responds to patients in crisis when needed

  • Knowledgeable of the mandating reporting process for adults, children and disabled persons.

  • Uses evidence-based practice with patients and/or family members for short term therapy when necessary

  • Facilitates or co-facilitates groups based on the Weight and Wellness practice needs

  • Provides Psychoeducation to patients and families as needed to support their weight loss goals

  • Maintains knowledge base of community resources and refers patients to these services as necessary

  • Attends team, departmental and practice meetings

  • Pursues ongoing education to maintain competence in practice

  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

    BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE:

  • Knowledge of area community resources

  • Ability to work effectively as an interdisciplinary team member with respect for the roles

    Education/Experience/Licenses/Technical/Other:

  • Education: Master of Social Work degree from an accredited graduate program required.

  • Experience : 3 to 5 years in a medical setting required. 2-5 years of acute hospital experience preferred.

    Demonstrates cultural competence in care delivery. Familiarity with Patient Centered Medical Home is a plus.

  • Certification/Licensure: Licensed or License eligible, as a LCSW in the state of MA

  • Software/Hardware: Microsoft products and Meditech. Proficient with Microsoft Office products, particularly MS Word and Excel.
