Job Description: To provide crisis prevention and management, monitoring, assessment, and service planning and coordination for up to 45 adults and children with intellectual or developmental disabilities (IDD), enrolled into the Home and Community-based Services (HCS) Medicaid Waiver Program through local HCS providers in Brazos, Burleson, Grimes, Leon, Madison, Robertson, and Washington Counties. Service Coordination involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual which is based on a Person Directed Plan (PDP). Will negotiate and facilitate the array of services and support needed to address the individuals personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. Is responsible for ensuring planned services and support are implemented and appropriately documented. Will advocate and ensure consistent interactions with other agencies. Education: Requires Bachelors or advanced degree with a major in a behavioral, social, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. Experience: Prefer experience working with individuals with IDD; must be able to work independently as well as with a team. Good organizational skills are a must. Hour of Work: 8:00am5:00pm; Monday through Friday; may require after-hours flexibility to meet the needs of the person in service/program. Licensure/Certifications: Must have reliable transportation and a good driving record, valid Texas drivers and must be insurable by Agency insurance. Physical Requirements: Ability to drive a vehicle; ability to communicate effectively; knowledge of case management principles, objectives, standards and methods, knowledge of Medicaid waiver program services and non-Medicaid waiver services; skill in determining individuals needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals, and ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
