*Job Description: As a LIDDA HCS Service Coordinator, provide crisis prevention and management, monitoring, assessment, and service planning and coordination to adults and children with intellectual or developmental disabilities (IDD), enrolled into the Home and Community-based Services (HCS) Medicaid Waiver Program through local HCS providers in Brazos, Burleson, Grimes, Leon, Madison, Robertson, and Washington Counties. Service Coordination involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual which is based on a Person Directed Plan (PDP) approach. The Service Coordinator will negotiate and facilitate the array of services and support needed to address the individuals personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. The Service Coordinator is responsible for ensuring planned services and support are implemented and appropriately documented. Will advocate and ensure consistent interactions with waiver provider and other agencies. Provides required monthly face to face (and collateral) contact monitoring of persons on caseload and documents activities as required by Texas HHSC-IDD Services and MHMRABV. *Education: *Requires a bachelors or advanced degree with a major in a behavioral, social, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice. *Experience: Prefer experience working with individuals with IDD; must be able to work independently as well as with a team. Must be computer literate, requires excellent verbal and written communication skills, requires excellent organization skills, including the ability to meet rigid timelines and manage multiple priorities.Good organizational and time management skills are a must.Knowledge of service coordination principles, objectives, standards, and methods; knowledge of Medicaid waiver program services and non-Medicaid waiver program services; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals. *Hours of Work: 8:00am5:00pm Monday through Friday; may require after-hours flexibility to meet the needs of the person in service/program. *Licenses/Certification: Must successfully complete Agency and TAC required IDD service coordination training within allotted timelines as applicable. *Physical Requirements: Ability to drive a vehicle; must have reliable transportation and a good driving record; must have a valid Texas drivers license and must be insurable by Agency insurance; ability to communicate effectively; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position. *Facility: LIDDA HCS Service Coordination Unit (Mary Lake), Bryan, TX.
