Crossroads Hospice & Palliative Care Hospice Social Worker (SW) - Full Time & PRN in Chesterfield, Missouri

The Hospice Social Worker assists patients and families by providing resources for care to enhance the patient's quality of life during adjustment, planning, and after end-of-life care.

Position Qualifications:

  • 1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care, or Acute Care environments.

  • LSW preferred

  • Master's Degree Required.

  • Licensed Social Worker Preferred

  • Medicare guideline experience preferred.

Job Responsibilities:

  • Assess and document patient/family social, emotional, financial, and psychological needs as well as resources on the Comprehensive Assessment.

  • Assist patient and/or family with establishing DNR, advance directives, and/or DPOA or Healthcare proxy as requested.

  • Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, and GIP as dictated by patient/family needs.

  • Work in both Hospice and Palliative care Programs.

  • PRN Availability to be on-call 2 weekends a month and pick up shifts as needed during the week

  • Full Time Mon to Friday 8:30 - 5:00 PM with an on call rotation
