CCMEP offers TANF and WIOA youth ages 16 - 24 the opportunity to complete their secondary education, achieve an advanced credential/certification and enter the workforce. The Case Manager provides assessment of services, documentation and coordination of service plan delivery to meet the performance expectations outlined in the CCMEP Scope of Work. Provide comprehensive assessment utilizing required tools and documents as outlined in CCMEP Scope of Work. Create Individual Opportunity Plans that address removal of barriers to ensure individual is success in achieving performance outcomes as outlined in the Scope of Work. Be creative in motivating individuals served to engage, retain and succeed in program services. Identify and create relationships and opportunities with community agencies to meet the needs of the individuals served. Comply with all program standards and expectations of CCMEP. Document progress of each individual served in OWCMS database as well as program database. This includes completion of the Comprehensive Assessment, Individual Opportunity Plan, offering of the 14 elements of service delivery, provision of supportive services and follow up activities.
