May implement prevention | | services over a varied group of topics including substance abuse, | | mental health, bullying, suicide and other areas as necessary. | +-----------------------------------------------------------------------+ | Position Description: | | | | - Identifies and assesses needs of callers/clients | | - Provide crisis intervention, referrals and linkage to community | | resources | | - Utilizes supportive techniques and/or educational methods that | | are appropriate to caller and topic | | - Completes service documentation accurately and timely in | | electronic health record | | - Provides both prevention and education services | | - Facilitates referral to and/or provides appropriate level of care | | - Develop, define and implement realistic and effective goals and | | plans for interventions | | - Collect, track and evaluate program data for fidelity and service | | quality | | - Adhere to professional standards as outlined by protocols, rules | | and regulations | | - Other duties as assigned | +-----------------------------------------------------------------------+

Qualifications
