Cumberland River Behavioral Health, Inc. PASRR Coordinator in Corbin, Kentucky

Job Title:   SGF/PASRR Coordinator (State General Fund/Preadmission Screening and Resident Review)  

Qualifications Include:  Kentucky License as a Registered Nurse (BSN or ADN), Licensed Professional Nurse (LPN) (supervisory/managerial experience and experience with individuals with developmental disabilities preferred) or Bachelor’s degree in a Human Services field; Submit to and pass a criminal record check and a drug screen test.

Hours:  Monday through Friday, 8:00 am until 4:30 pm.  Different hours may be required with the job or may be adjusted by Supervisor.

This position reports to the D/ID Director and/or Assistant D/ID Director.

Summary of Job Duties:  The SGF/PASRR Coordinator will work closely with the D/ID Director and Assistant D/ID Director to ensure the Program is operating effectively.

Essential Duties and Responsibilities: 

 

  • Maintain client record documentation to reflect items used to make determination of eligibility for SGF;

  • Communicate in all directions in a manner that is positive, productive, and supportive of teamwork, innovation, and collaboration;

  • Create and maintain a regional culture of inclusion focused on growth and improvement;

  •  Work collaboratively with management teams to ensure quality service delivery;

  • Attend regular staff meetings;

  • Support and work within the mission and strategic plan of the organization;

  • Ensure compliance with relevant laws, regulations and standards. e.g., state contracts, Joint Commission, etc.;

  • Work collaboratively with other region-wide programs to determine services offered in the region;

  • Ensure that clients utilize other resources within the region in a manner that optimizes the service delivery system;

  • Complete assessments to determine eligibility for ongoing services and maintain client plan of care that is reflective of needs as identified by interdisciplinary team;

  • Develop crisis prevention plan and connect participants with additional community based services to help prevent institutionalization;

  • Complete required documentation and training;

  • Ensure that all staff who bill for PASRR services are trained and certified in PASRR policies and procedures;

  • Ensure that all evaluations meet federal and state guidelines for determinations, content, and timeliness;

  • Notify the Department of Behavioral Health, Developmental and Intellectual Disabilities (DBHDID) of any changes in staffing or staffing credentials;

  • Distribute manual revisions and related information in a timely manner, upon receipt of information from the DBHDID and/or DMS;

  • Regularly review completed evaluations and records for compliance with policies and procedures, including timelines and content;

  • Be available for consultation to staff;

  • Coordinate local training for PASRR staff, nursing facilities, and others as needed;

  • Ensure that required forms and information are submitted timely to Kentucky Level of Care Services (KLOCS) and DBHDID;

  • Ensure that all documentation and billing information are submitted timely to DBHDID;

  • Complete all DBHDID trainings and maintain current knowledge of PASRR requirements and best practices;

  • Ensure tasks, evaluations, and determinations are completed and/or uploaded into KLOCS in a timely manner and without delay;

  • Participate in DBHDID quarterly PASRR meetings and DBHDID sponsored trainings;

  • Ensure that individuals approved for Specialized Services receive those services timely;

  • Ensure that specialized services are provided in accordance with federal and state regulations;

  • Supervise PASRR case managers and Direct Support Professionals, including those who are subcontracted employees;

  • Ensure that quarterly reports are accurate and submitted timely to DBHDID;

  • Conduct professional, individualized and comprehensive clinical evaluations in accordance with state and federal regulatory requirements;

  • Conduct face to face and phone interviews;

  • Gather comprehensive medical, psychosocial, and mental health information including medical records and supporting documents needed to make PASRR decisions;

  • Document the individual’s disability status history and unique needs and optimal care strategies;

  • Assess all information to determine level of care, the need for specialized services, and to make recommendations;

  • Complete onboarding requirements and trainings for KLOCS;

  • Other duties may be assigned as needed.

    Required Job Skills:

  • Typewriting and Computer skills.

    Physical Demands and Work Environment: 

  • The physical demands required to successfully perform this job include being able to push, pull, bend, stoop, stand, walk and lift up to 20 pounds.  Reasonable accommodations will be made to enable the individual with disabilities to perform these essential functions.

  • The employee will normally work indoors but may on occasion be required to work outside or at another location if necessary.  The noise level in the work environment is mild to moderate.

  • The position may require duties to include routine or reasonably anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials.

  • Hours of work are structured 7.5 or 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands.

  • Participation in annual training regarding infection control and safety in the environment including hazardous waste and blood borne pathogens will be required.

  • All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned.
