Job Summary and Qualifications

Position Summary:

Responsible for documenting and gathering information about a patient for a provider visit. Prepares and assembles medical record documentation/charts utilizing all available resources including but not limited to updating patient history, summaries course of care, history of present illness, organizing and sending all documentation to the provider for review assisting in the delivery of health care and patient care management under the direction of a physician or mid-level practitioner. Ensures medical record compliance by self-documentation attestation to include scribe name, title and provider working for as well as specific date and time entry.

Essential Functions include but are not limited to:

• Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices.

• Record patients' medical history, history of present illness, summarizing course of care and information such as test results in medical records.

• Other clerical duties and tasks to improve work flow

• Computer skills with Microsoft Word and Excel; word processing skills.

• EHR (eClinicalWorks preferred), familiarity with EPIC, Meditech

• Basic understanding of medical terminology and abbreviations commonly used.

• Promotes internal and external customer satisfaction.

• Builds and promotes a culture of service excellence and continuous improvement.

• Attendance: works as scheduled and is compliant with Attendance and Tardiness Policy.

• Complies with hospital policies including HIPAA

• May accompany provider during patient visits to assist in documenting the provider assessment and exam

Education & Qualifications:

• Required: prior scribe or similar role, knowledge of medical terminology.

• Preferred: High school diploma or GED

• Must exhibit quality communication skills and written communication/documentation
