Description We are offering a short-term contract employment opportunity for a Patient Care Coordinator in Fairfield, New Jersey. This role will be vital in the healthcare industry, coordinating patient care and ensuring smooth communication between various stakeholders.

Responsibilities:

• Coordinate nursing care for specific cases based on referrals, including determining coverage and liaising with nurses and agencies

• Communicate effectively via phone calls, emails, and sometimes texts to arrange care

• Participate in daily team meetings to discuss and prioritize cases

• Once a nurse is confirmed for a case, reach out to the patient to confirm details such as address, phone number, and frequency of visits

• Escalate issues to management as necessary for resolution

• Maintain regular communication with physicians, patients, caregivers and other relevant parties

• Manage patient scheduling and procedure scheduling efficiently

• Navigate and utilize insurance processes as required to facilitate patient care

• Ensure all actions are in line with company policies and procedures, maintaining a patient-centric focus at all times. Requirements • Must have a minimum of 1 year of experience as a Patient Care Coordinator or similar role

• Proficiency in patient scheduling and procedure scheduling is essential

• Knowledge and experience in dealing with insurance matters related to patient care

• Excellent communication and interpersonal skills

• Ability to work effectively in a team and collaborate with healthcare professionals

• Strong organizational skills and ability to multitask

• High attention to detail and accuracy

• Must have a patient-centric approach, ensuring the highest quality of care and service

• Proficiency in using healthcare software and systems

• Understanding of medical terminologies and procedures

• Ability to handle sensitive information confidentially

• Must have the ability to work under pressure and handle emergency situations effectively.
