County of Gaston Public Health Nurse II in Gastonia, North Carolina

The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform.

  • Clinic Duties: Utilizes the Nursing Process to assess, interview, triage, educate and implement the plan of care. Examples: complete vital signs, interviews, and patient/family education. Administers immunizations per CDC guidelines and documents in NCIR. Assists with procedures and provides treatments per clinic guidelines. Participates in all clinical programs. Family Planning, STD, CD, Immunizations, TB, and other areas as needed. Reviews encounter accuracy. Other duties as assigned.
  • Leadership: Role models positive teamwork and professional communication. Performs duties of Charge Nurse, as back-up when needed, too include assessing and directing clinic flow, general problem solving, delegating duties and tasks to appropriate staff, preparing daily schedule, including any special clinic projects/programs.
  • Assigned Follow-up: Nursing team members will be assigned specific tasks by Nursing Supervisor or Charge Nurse to assure that contract addenda requirements and clinic specific needs are met. Examples: Lab review and follow-up of positive results, medication and LARC ordering and inventory, AED and Code Cart monitoring, immunization refrigerator temperature monitoring, stocking literature and Depo bags, follow-up of missed appointments, positive pregnancy tests and other miscellaneous tasks as n
  • Referrals: Complete pre-authorization process, make appointments for needed services as per orders, contact patient/parents via phone or mail to advise of recommended plan of care, document in patient record per policy. Completes follow-up to assure that plan of care is completed. Contact attempts and closing of patients to follow-up will be completed per policy.
  • Triage: Follow-up with patient calls, assess problems, provide medical advice, schedule appointments, assess and triage walk-in patients based on established procedures, protocols and standing orders; consults provider as indicated.
  • Quality Assurance/Quality Improvement: Complete monthly program audit of a least 5 records. Collaborates and communicates with co-workers, medical providers, supervisor, PHS Management, and other departments to ensure best practices within the department. Provides input and feedback to the supervisor regarding policies and procedures and clinic operations.
  • Education: Participates in yearly competence activities for PHS Division staff. Participates in in-services, trainings and meetings. Serves as a preceptor to new staff. Completes required competencies per NC Board of Nursing to ensure compliance with nursing license.
  • Disaster Response: Assists the community during emergencies such as natural disasters, pandemic or bio-terrorism events by delivering services in the field as needed. Participates in staffing emergency shelter, administering vaccinations to the community and providing nursing care to assure community safety.
  • Carryout Performance Management (PM) roles and responsibilities, including identifying and communicating performance improvement opportunities and adjusting performance and practice to achieve established PM goals.
  • Community: Delivers nursing services through community based clinics, home visits, case management, and community outreach programs. Provides education and resources to underserved populations. Administers vaccinations to the community and providing nursing care to high risk populations within the community.
Minimum Qualifications

Graduation from an accredited college or university with a Bachelor's degree in Nursing which includes a Public Health Nursing rotation; OR a Associate's Degree or Diploma in Nursing: one (1) year of professional nursing experience required. OR equivalent combination of education and experience.

No previous experience required for Bachelors Degree Nurses.

Must be licensedby the tate of North Carolina as an RN (Please include copy of Nursing License with application).

A valid BCLS CPR certification required.

Must have a valid driver's license.

Additional Information

Minimally qualified applicants will start at 25% of the salary range:$29.84/hr.
