DESCRIPTION OF WORK As an Aging Care Manager 2, you will be responsible for providing assessment, care planning, service coordination, and performance of ongoing care management tasks for vulnerable consumers who are in need of protection. Consumers include those in need of mental health case management services who may face and present unique challenges in maintaining a safe and healthful independent lifestyle. Sound judgement, precise compliance with time frames, and strict adherence to procedures are essential to protect vulnerable clients form dangerous or life-threatening situations. Work Schedule and Additional Information: Full-time employment Work hours are 7:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. This position requires on call hours. QUALIFICATIONS Minimum Experience and Training Requirements: Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.

Hiring preference for this vacancy will be given to candidates who live within Pike County. If no eligible candidates who live within Pike County apply for this position, candidates who reside in other counties may be considered. You must be able to perform essential job functions.
