Providence St. Joseph Health Physical Therapist in Issaquah, Washington

Description This position is responsible for therapy evaluations, treatment planning, implementation and discharge planning for clients in the home setting in accordance with established facility/agency policies while maintaining the highest degree of quality client care. Required Qualifications: Bachelor's Degree - Physical Therapy from an institution approved by theState board. Washington Physical Therapist License upon hire. National Provider BLS - American Heart Association within 30 days of hire. National Provider Identifier (NPI) number within 30 days of hire. Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request (SCS Home Health only). See policy for additional information. 2 years - Therapy experience with adult physical disabilities. Preferred Qualifications: 1 year - Therapy experience in home care or in related rehabilitation setting. Experience with the geriatric population. Why Join Providence?
