UH Lakewood Medical Center is located between Kansas City and Lee’s Summit, Missouri, offering residents of Eastern Jackson County access to a modern community hospital with a continuum of outpatient services designed to meet the needs of families. UH Lakewood specializes in high-risk maternal and child care, and serves as one of the few Kansas City metro area institutions accredited by The Joint Commission in Orthopedics and Palliative Care. It also is home to a 188-bed, fully accredited long-term care center.

UH Behavioral Health is the KC metro area’s largest, most comprehensive provider of mental health services. We also offer psychiatric inpatient care, and we deliver therapeutic services through our community outreach clinics. Job Description

The Health Home Healthcare Navigator is responsible for the coordination of healthcare services through interdisciplinary collaboration to help achieve optimal patient outcomes. This includes scheduling and managing appointments, guiding patients to the appropriate area of care and/or service and working collaboratively with the organization and University Health clinical/non-clinical personnel to identify barriers and find solutions to create a seamless experience for patients. Position demands a highly customer service oriented professional to communicate with patients requesting services within the organization exceeding the expectations of the customer and promoting advocacy through effective

relationship management. A fundamental role of this position is working to build and sustain strong relationships with enrolled patients, caregivers, physicians, nurses and support staff throughout the organization to accommodate patient requests and keep enrolled patients coming to University Health.

Minimum Requirements

Bachelor’s degree or equivalent combination of education and relevant work experience

Outgoing personality and excellent verbal and written communication skills

Ability to acquire and maintain strong knowledge of resources and services within the organization, University Health and community health related organizations

Ability to present a calm, professional demeanor in a demanding, high stress environment, consistently demonstrating good judgment and superior customer service and problem solving skills.

Demonstrated ability to manage patient concerns with tact and confidentiality.

Excellent interpersonal skills with ability to work with a wide range of people from diverse cultural, socio-economic and professional backgrounds with demonstrated experience in proactive engagement with patients, clients, and guests.

Autonomous self-starter and team-player

Expert ability to navigate and use scheduling software, utilizing current clinic guidelines.

Ability to use Excel worksheets or other database to document contacts/referrals and create reports of activities.

Professional appearance and ability to represent the organization and/or University Health in a way that increases patient satisfaction

Preferred Qualifications

Background in health care, sales, hospitality, or customer service

Ability to show proven success in creating and maintaining relationships with a positive outcome

Proven ability to problem-solve to create positive outcomes
