Description

Job Purpose :

This is a technical position that involves the direct care of adults with severe and persistent mental illness. This position assists resident’s in attending their scheduled community appointments, advocates on the residents' behalf during appointments, assists residents in meeting their daily living needs, ensures a standard of cleanliness in the resident’s living environment, and facilitates recreation and socialization among residents and within the community.

Reporting Relationships

This position reports to a Residential Clinical Supervisor. This position has no direct reports.

Essential Job Functions :

  1. Provides transportation for residents to appointments, community outings, personal needs shopping, etc. as needed.

  2. Advocates for residents during appointments as needed, to ensure the resident’s needs are met.

  3. Assists consumers in learning independent living skills, which are defined in the resident’s Individual Service Plan. (i.e. shopping, community safety, laundry, housekeeping, cooking, time and money management.). As well as encouraging management of symptoms through support services and education.

  4. Documents resident appointment summaries and behaviors in their progress notes.

  5. Coordinating client care, doctor recommendations, and doctor orders with Residential Coordinator and WHCG RN.

  6. Monitors consumer medication adherence as directed by their physician.

  7. Reports to Residential Clinical Supervisor, WHCG RN, or physician any changes in client behavior and functioning.

  8. Ensures that residents maintain acceptable standards of personal hygiene.

  9. Ensures that the house is clean at all times and complies with DHS 83 CBRF regulations.

  10. Completes monthly activity calendars and prepares materials for each assigned CBRF including resident and BHT staff feedback.

  11. Provides direct care (BHT) coverage in the CBRF during BHT vacancies or PTO.

  12. Assists with needed supply/medication deliveries to the CBRF’s.

  13. Attends and participates in staff meetings, in-service training seminars and conferences as required.

  14. Attends required annual CBRF trainings and is able to successfully pass competency testing.

  15. Understands and adheres to established policies and procedures and DHS 83 rules.

Qualifications

Required Technical Skills, Experience, Education, and Credentials :

  1. Bachelor’s Degree or related experience is preferred. High School Diploma or equivalent is required.

  2. Minimum of1years’ experienceworking with adults with severe and persistent mental illness.

  3. Must be able to function with minimum supervision.

  4. Professional workplace appearance and conduct; friendliness and courtesy to consumers, coworkers and vendors.

  5. Effectively communicates and develops good working relationships with co-workers.

  6. Reliability in reporting to work regularly and on time.

  7. Must possess good communication, listening, writing and organizational skills.

  8. Must complete CBRF training program as outlined by DHS 83.

  9. Must be able to use appliances such as stove, microwave, washer/dryer, etc.

  10. Must be able to read the label on medicine bottles and understand the directions on the labels.

  11. Must be able to clean up bloody/body fluid spills in accordance with OSHA and agency policies.

  12. Must be able to evacuate residents in two minutes or less and assist residents in evacuation as dictated by DHS 83.

  13. May have to provide group home coverage during times of direct care vacancy or scheduling emergencies

  14. Motor vehicle record acceptable to program and/ or contractual requirements.

  15. A valid Wisconsin driver's license, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage”.

Work Environment and Physical Requirements

The work environment and physical demands described here are representative of those that an employee may experience or must do to successfully perform the essential functions of the job:

  1. Work is performed largely in a group home setting. Group homes are open 24 hours per day 7 days a week.

  2. Hours of work will vary and will be set in conjunction with the designated supervisor. Management has a right to move staff to a group home that is not their home location, if they are properly trained, for emergency staffing needs.

  3. Must be able to communicate complex thoughts and instructions both in writing and verbally.

  4. This position is responsible for the supervision and safety of consumers in both normal program settings as well as a variety of settings in the community. As such, this position must be able to observe consumer activities, detect areas of danger, distinguish when consumers who are non-communicative are in need of personal cares, and converse clearly with consumers and other people in the community on a frequent basis.

  5. Ability to perform well in a fast-paced work environment.

  6. On occasion must be able to push/pull/lift/move objects weighting at least 50 pounds.

  7. This position may work in a variety of indoor and outdoor work environments which may include, various outdoor weather conditions, loud noises, and fluctuating temperatures and the occasional use of stairs.
