Amergis Registered Nurse (RN) - Public Health NFP in Los Angeles, California

The Nurse-Family Partnership (NFP) is a nationally acclaimed nurse home visitation program that provides 2 ½ years of intensive home visitation services to pregnant clients who are living in poverty. The evidence-based model has been shown to foster healthier pregnancies, enhance the health and development of children, and improve the life course of the parent and the family by encouraging self-sufficiency. NFP- Los Angeles (NFP-LA) is currently hiring Temporary Public Health Nurses for SPAs 2 and 6. (www.nursefamilypartnership.org)

Minimum Requirements

• Must hold a current Public Health Nurse Certificate

Examples of Job Functions

• Provide home visitation to a variety of areas in Los Angeles County

• Adhere to NFP guidelines in serving high risk pregnant clients/youth

• Utilize a client-centered and relationship-based approach to patient care

• Establish long-term relationships with the clients and their families

• Trained and certified in assessment techniques

• Provide mental health screening as trained

• Outreach and partnership with community agencies

• Collect and submit data as required by NFP in a timely manner

• Participate on community/county committees/projects to improve NFP services and customer satisfaction

• Maintain a caseload of 25 active families.

Desirable Qualifications

• Home visiting experience

• Passionate about serving the high-risk pregnant clients and their children

• Fluent in Spanish preferred

• Willing to learn and able to travel to be trained when required

• Ability to address mental health issues and refer clients when needed

• Proficient in utilizing Microsoft Office and digital technology

• Possessing strong organizational, communication and time management skills

• Excellent ability and knowledge to work within a multidisciplinary team
