What You'll Do

 

This Community Health Worker I (CHW-I) provides health education, health care advocacy, and health care system navigation with homeless families and individuals at several sites in the region, improving access to health services for this population. The CHW-I also conducts focused community outreach and provides 1:1 health insurance enrollment assistance through the Health Benefit Exchange. Lived experience and demonstrated community involvement highly desirable. This position will receive significant on the job training and there are potential advancement opportunities with long term tenure.

 

This position has a social justice component that requires critical thinking through the lens of racism and intersections with poverty. Knowing the core principals of antiracism and grounding those principles in everyday work are required job skills and core values.

Community Health Worker Responsibilities
  • Maintains a schedule of availability with community partners to provide services to program participants via telehealth in King and Snohomish counties
  • Assesses health coverage and resource needs for program participants
  • Provides health care navigation and coordination services including social services that address the social determinants of health
  • Facilitates WA Apple Health (Medicaid) and Qualified Health Plan enrollment 
  • Provides program participants with health education and advocacy about managed care
CHW-I Requirements
  • One year of direct social service or health care related experience with one year of experience with low-income and/or homeless families

  • A Bachelor's degree in social services or a related field with at least two years of experience with low-income and/or homeless families

[AND]{.underline}

  • Lived experience and/or an active member of the community programs we serve
  • Knowledge and experience related to domestic violence, homelessness, mental health, or case management
  • Familiarity assisting with health insurance enrollment and access to medical, dental, vision, specialty care
  • Ability to communicate and collaborate across agencies, organizational cultures
  • Demonstrated knowledge and skills of basic computer competencies such as emailing, calendaring, entry-level data entry and familiarity with Microsoft Office product
  • Demonstrated commitment to anti-racist principles and commitment to working with people who are homeless or otherwise at risk
  • Experience working with communities of color and people from different cultures than your own
  • Ability to travel to various work sites, as needed
Hours, Rate, and Benefits

Hourly Rate: $25.00
