Acentra seeks a RN Clinical Assessor to join our growing team.

Job Summary:

Embark on a transformative career journey with Acentra Health as a Nurse Assessor. As a key member of our dedicated team, you will play a crucial role in assessing client needs within the community and submitting assessments to assist with program eligibility. This position allows you to significantly impact individual lives by assessing client needs in the community and providing the information for submission to assist with program eligibility.

In your role as a Nurse Assessor, you will leverage your expertise to accurately assess client needs and services within homecare settings. Your assessments will be instrumental in securing better health outcomes, providing critical support that aligns with the specific needs of each individual.

This is a PRN Hybrid role that will entail working from your home office while also dedicating a portioneach day to fieldwork. This involves travel to clients within the coverage area in New Hampshire. Some of the coverage areas include but are not limited to; Manchester, Nashua, Merrimack, Bedford, Hudson, Goffstown, Milford and Amherst. Travel expenses, including mileage will be reimbursed at the federal rate.

Job Responsibilities:

  • Conduct clinical assessments of patients by meticulously reviewing medical records, consulting with collateral sources, and analyzing available data to ensure accurate evaluations, recommendations, and determinations.

  • Oversee the adherence to professional standards in determining the Level of Care required, ensuring all assessments are aligned with best practices.

  • Collaborate effectively with office staff to streamline workflows, enhancing the efficiency and effectiveness of our service delivery.

  • Engage in meaningful interactions with healthcare professionals, patients, their families, and other support systems to gather insights and provide holistic care.

  • Provide timely annual reassessments to identify opportunities for improvement or necessary changes in care plans, facilitating continuous care optimization.

  • Utilize your independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.

  • Develop informed Level of Care recommendations based on thorough clinical evaluations, ensuring compliance with contractual and regulatory standards.

  • Build positive, professional relationships across all levels of operation, acting as a liaison to foster effective teamwork and facilitate the review process within and beyond the organization.

  • Conduct assessments in various environments, including nursing homes, hospitals, and community settings, to understand and address the diverse needs of those we serve.

  • Leverage advanced technology, utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.

  • Additional Accountabilities This list may expand to include additional duties deemed necessary by management.

Required Qualifications/Experience:

  • Must have an active Registered Nurse (RN) license in the State of New Hampshire with a minimum of 2 to 3 years of experience.

  • Completion of a program from an accredited institution, Associate Degree, or Bachelor’s Degree in Nursing, equipping you with a solid educational foundation in the nursing field.

  • Demonstrated experience in clinical assessment and critical thinking abilities, showcasing your capacity to make informed decisions and provide high-quality care.

  • Exceptional verbal and written communication skills, enabling you to articulate complex medical information clearly and effectively across diverse platforms and audiences.

  • A proven track record of working effectively within a team-oriented environment, contributing to a cohesive and supportive workplace culture.

  • Demonstrate unparalleled flexibility and organizational skills, adept at managing multiple priorities with efficiency and precision.

  • Proficiency in Microsoft Excel, Word, and Outlook, coupled with the ability to utilize computer equipment and web-based software efficiently, ensuring a seamless workflow.

  • Competent in communicating effectively with individuals and groups through spoken, written, and electronic media, enhancing engagement and understanding.

  • A meticulous approach to work, with the ability to prioritize effectively and execute tasks with precision and timeliness.

  • Ability to meet local travel requirements, ensuring the completion of assessments and adherence to deadlines, demonstrating commitment and flexibility in service delivery.

Compensation

The compensation for this role is $35.00 to $37,00 per hour.

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
