Position Summary:
nvestigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

ESSENTIAL FUNCTIONS:
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Drag Edit Delete
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Drag Edit Delete
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Drag Edit Delete
Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Drag Edit Delete
Performs Medicaid screenings as indicated for NHP. Drag Edit Delete
Assists patients with Safety Net applications when needed.
Minimum Qualifications:

Required Skills Licenses: State of Michigan Licensed Master's Social Worker (LMSW) Certifications: Basic Life Support Health Care Provider (BLS-HCP) Drag Edit Delete
Minimum Work Experience 6 months related experience 2 years related experience (Preferred)
