Responsibilities POSITION SUMMARY: The High Value Care Coordinator monitors value/quality based reimbursement programs offered by medical group commercial payers and CMS and works in collaboration with the Practice Administrator and/or Practice Manager at each location to close gaps and increase value based reimbursements. The compensation range for the role is $58,469- $73,086 annualy Garnet Health System provides a compensation range to comply with the New York State law on Salary Transparency in Job Advertisements. The range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits. When determining a team member’s compensation and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity). Qualifications POSITION QUALIFICATIONS: Minimum Education: High School\GED degree required (Associate\Bachelor preferred) Minimum Experience: 3-5 years of experience in health care settings, ideally in the areas of direct patient care, case management, care coordination, patient outreach, front desk\clinic reception, and\or quality improvement Language Skills: Ability to read, write and speak English. Reasoning Ability: Able to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills: To perform this job successfully, an individual should have knowledge of electronic medical records (experience in Epic preferred) , and Microsoft Office Suite including Excel, MS Word, PowerPoint, Outlook etc. Proficient in data analysis and manipulation (Excel summary tables, charts, pivot tables) Workplace type On-site
