GENERAL OVERVIEW:

Responsible for accurate and timely completion of Inpatient Rehab Facility-Patient Assessment Instrument (IRF-PAI) with the interdisciplinary team, gathers and inputs data, transmits data to Medicare. Oversees all aspects of data collection for the IRF-PAI, and ensures the presence of supporting documentation. Interprets Policies and procedures and acts as primary resource for immediate problem solving in regards to CARE scoring and regulatory requirements for inpatient rehabilitation. Provides education to the interdisciplinary team in regards to Item completion of the IRF-PAI. This role serves as a key member of the Inpatient Rehabilitation leadership team in helping the facility monitor/maintain quality outcomes and CMS requirements.

ESSENTIAL RESPONSIBILITIES

  • Responsible for the accurate coordination and completion of the IRF-PAI and quality data in the computer, submission, tracking and reporting as it related to aspects of the IRF-PAI and quality reporting program process. (50%)

  • Performs compliance audits and monitors CMS compliance. (20%)

  • Provides staff training and feedback on proper and accurate scoring and documentation of Care tool and other IRF-PAI items and assures Competency. (20%)

  • Coordinates the interdisciplinary Team conference. (5%)

  • Assists in outcome monitoring and performance improvement. (5%)

  • Other related duties. (5%)

QUALIFICATIONS:

Minimum

  • High School/GED

  • 3 year's in Healthcare

  • Registered Nurse (PA) OR Physical Therapist OR Occupational Therapist OR Speech Language Pathologist

  • Basic Computer and data entry skills

  • Clinical Assessment and analysis skills

  • Must be able to communicate effectively with Patient's families, interdisciplinary team members, and regulatory groups through written and verbal communications

  • Excellent organizational skills and ability to work independently and within a group setting.

Preferred

  • Bachelor's Degree

  • 1 year in Acute Inpatient Rehabilitation
