The Quality Director for Post Acute Care Services is a key player in enhancing patient outcomes by leading quality improvement initiatives across various disciplines within home health and hospice. This role involves analyzing and managing diverse data sets, including clinical, financial, quality, utilization, and outcomes data, to identify areas for improvement and implement effective solutions. The specialist ensures compliance with regulatory requirements, including those set by The Joint Commission (TJC), CMS, and other accrediting bodies, through timely data submission and progress tracking on core measures. Additionally, the position requires developing and maintaining software programs, spreadsheet macros, and database structures to support quality improvement projects. As the administrator for HHCAPS, the specialist oversees all regulatory aspects of home health and hospice services, updates data on the Care Compare tool quarterly, and conducts regulatory audits to ensure alignment with industry standards.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Associates degree in Nursing, Healthcare Administration, Business, or a related field. EXPERIENCE: 1. 3-5 years of experience in quality improvement and data analysis. 2. Experience with regulatory compliance, including TJC and CMS requirements, is highly desirable
PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor's degree in Nursing, Healthcare Administration, Business, or related field. EXPERIENCE: 1. Experience in home health and hospice.

Analyze and manage clinical, financial, quality, utilization, and outcomes data to identify areas for improvement in patient care.
Lead quality improvement initiatives across multiple disciplines within home health and hospice services.
Ensure compliance with The Joint Commission (TJC), CMS, and other accrediting bodies by submitting required data in a timely manner.
Track progress on core measures and introduce new quality indicators to meet regulatory standards.
Develop and maintain software programs, spreadsheet macros, and database structures to support quality improvement projects.
Facilitate multi-disciplinary teams in using data to drive improvements in patient outcomes.
Serve as the administrator for HHCAPS, overseeing all regulatory aspects of home health and hospice services.
Update and maintain data on the Care Compare tool on a quarterly basis.
Conduct regulatory audits to ensure compliance with industry standards and guidelines.
Collaborate with internal stakeholders to develop and implement strategies for continuous quality improvement.
Maintain report writing capabilities on all relevant systems to support data-driven decision-making.
Provide guidance and support to staff in understanding and meeting quality and compliance requirements.
Monitor and report on the effectiveness of quality improvement initiatives, making recommendations for adjustments as needed.
Stay current with changes in regulatory requirements and best practices in home health and hospice care.
