This is a support position aiding case managers in the performance of specified job duties.
ESSENTIAL FUNCTIONS:
- Complete referrals as needed for the child(ren), caregiver(s), and parent(s) within 5 business days of identified need.
- Complete and submit Diagnostic and Evaluation Requests as well as all required documentation to the Utilization Management Department within 5 business days of the request.
- Make and document in FSFN monthly contact with all service providers; a detailed summary to be sent to assigned care manager and their supervisor within 3 business days of the contact.
- Complete Interstate Compact for Placement of Children/Out-of-County Services requests with all required documents within 2 weeks of request. Submit the packet to the POC for processing once reviewed and approved by the assigned case manager and their supervisor.
- Maintain and document in FSFN monthly contact with the OCS worker. A detailed summary will be provided to assigned case worker for review.
- Obtain Medical/Dental records for the file as needed. Scan the reports into FSFN once reviewed and initialed by the assigned case manager. Complete referrals and schedule appointments follow-up as mandated in the medical/dental reports.
- Review Court Orders (Docket sheet) and create a tracking mechanism to ensure all tasks are completed within 2 weeks of receipt of the order.
- Compile all required documents for SIPP/STGH admission within 2 weeks of request.
- Compile subsidy packets for case manager and supervisor review within 2 weeks of request.
- Assists case managers with follow-up and timely documentation of Comprehensive Behavioral Health Assessments (CBHA) tasks by established timeframes.
- Works in conjunction with the case management and adoption teams to prepare cases for transfer to the adoption units, to include record review in FSFN, records requests, including timely follow-up and uploading of documents in FSFN.
- Assists case managers with timely scheduling of medical and dental appointments, as well as documenting these appointments in FSFN within two business days of occurrence.
- Provide job shadowing and mentoring for new staff as requested.
- Other duties as assigned.
QUALIFICATIONS:
Education/Experience:
- High School diploma or General Education Degree (GED) with 6 months to one year experience required. Associates degree and one year experience in the human services field preferred.
Licensure/Certifications:
- Employer conducts background check
- Valid Florida Driver's License for required travel
KNOWLEDGE, SKILLS and ABILITIES

- Interpersonal and communications skills (verbal and written).
- General office equipment such as phone, copier, computer, etc.
- Ability to occasionally work a flexible schedule, including evenings or weekends.
