Social Worker-Clinical

The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

Social Worker-Certified

The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.

Position Details:

This position will work in ED Care Management primarily at Layton Hospital with some shift potential at McKay Dee Hospital. There are on call shifts at McKay Dee Hospital attached to this position as well. ED Care Management is a supportive service who responds to Trauma’s and Critical Response situations. We meet face to face with patients and family during crisis situations to provide support, resources, and arrange discharge services. We work with patients who frequent the ED to help find resources to better meet their needs. We work closely with the team of physicians and nurses to ensure safety of patients once they leave the Emergency Department.

Shifts for this role are Thursday and Friday at Layton Hospital from 1:00 pm – 9:30 pm. Other shifts areavailable at McKay Dee are from 6:00 am – 4:30 pm and 4:00 pm – 2:30 am. On Call shifts are 1:30 am – 6:00 am.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience and licensure.

SSW licensure: 27.19 – 41.94

CSW licensure: 29.89 – 46.14

LCSW licensure: 32.90 – 50.76

Minimum Qualifications:

Bachelor's Degree in Social Work or higher. Education must be obtained from an accredited institution. Degree will be verified.

Current State Licensure as an SSW, CSW, or LCSW.

Computer Skills, i.e., email, word processing.

Excellent written and verbal communication skills.

Preferred Qualifications:

Experience with patients, families, and treatment teams.

General knowledge of community resources

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

  • and -

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

  • and -

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

  • and -

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

May be expected to stand in a stationary position for an extended period of time.
