The role you’ll contribute:

OurLegacy is designated by the Centers for Medicare and Medicaid Services (CMS) as the only organ procurement organization (OPO) in east central Florida and is accredited by the American Association of Tissue Banks (AATB) as a tissue recovery agency. OurLegacy’s scope of operations extends to each of the following hospitals within the Advent Hospital system: Altamonte, Apopka, Celebration, Daytona Beach, Deland, East Orlando, Fish Memorial, Kissimmee, New Smyrna Beach, Orlando, Palm Coast, Waterman, and Winter Park. Additionally, CMS has designated OurLegacy to provide organ donation services to all non-AdventHealth acute care and critical access hospitals and free-standing emergency rooms in the following counties: Brevard, Flagler, Indian River, Lake, Okeechobee, Orange, Osceola, Seminole, Sumter, and Volusia.

The Surgical Services Coordinator (SSC) supports the mission, goals, and strategic plan of OurLegacy by overseeing all deceased donor organ perfusion and preservation processes and is responsible for the intraoperative leadership of a team of clinical professionals while managing the recovery and preservation of all solid organs. The SSC must have advanced knowledge of human anatomy, problem solving and technical expertise in surgical procedures.

Each organ donation can present a new set of unusual circumstances that must be independently identified and assessed in real-time to ensure the successful recovery of a transplantable human organ. The SSC role requires a high degree of autonomy, organization, judgment, ingenuity, and ability to adapt, and problem solve in the face of unforeseen surgical, interpersonal, and logistical problems. Innovative problem solving must happen in a rapid and decisive manner to lessen the potential for adverse implications that could negate the ability to transplant one or more organs from a deceased donor.

The SSC is expected to identify issues that arise in the organ recovery process, to analyze, compare and evaluate various courses of action to address those issue and to exercise judgment and authority to make independent decisions, free from immediate direction but within the scope of their responsibilities. Primary activities and decision-making authority are predominantly performed independent of physician oversight.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES: (Extensive Training Program Provided)

Clinical Responsibilities

• Stays informed about development and introduction of new supplies and materials used in the organ recovery, preservation and transportation process and works with team to make adjustments in accordance with best medical practices.

• Ensures all aspects of the organ recovery process are performed to optimal levels in accordance with regulatory policies, and organizational procedures.

• Ensures stewardship of all donated organs and provides access to transplant programs by performing all perfusion, packaging, labeling and regulatory documentation in an expedited manner, and without error.

• Exercises independent judgement and problem solving capabilities when circumstances require to ensure effective recovery, preservation and transportation of donor organs.

• First Assists AdventHealth Transplant surgeons or visiting surgeons from other transplant programs with anatomic dissection and surgical procurement of organs for transplant, research, or education according to established protocols and procedures. The donor surgery features qualified transplant surgeons performing the procedure up to the point at which organs are removed from the donor. After organs are placed on the back table, the surgeon exits the procedure, and the SSC assumes primary responsibility for the organs until they are labeled and packaged for transport.

• Independent of surgeon participation, dissects, evaluates, and packages organs and tissues removed from deceased donors.

o Performs aseptic organ preservation by flushing organs with appropriate preservation solutions, packaging and labeling organs, surgically dissecting tissue typing material (mesenteric lymph nodes and spleen) and blood vessels in accordance with regulatory requirements and OurLegacy Policy. The SSC is accountable to accurately distinguish lymph nodes from mesenteric fat which ensures that successful compatibility testing between donor and recipient can be achieved.

o Independent of physician oversight, evaluates the physical condition of deceased donor kidneys after removal from donor; performs dissection of adipose tissue from the donor kidney without damaging the organ; reviews anatomy and independently assesses appropriateness for placement on pulsatile perfusion device and advises surgeon of decision outcomes. Identifies and assesses organ anatomical abnormalities that, in the opinion of the SSC, require notification to the transplant surgeon and/or pathologist for further investigation.

o Performs dissection of perinephric fat to determine the vascular configuration of donor kidneys. Identifies the number and location of renal arteries and veins. Communicates this information to the organ procurement coordinator so it may be relayed to receiving transplant programs so they may adequately prepare for the implantation procedures.

o Independently performs all aspects of kidney pulsatile preservation including cannulation, machine set-up and parameter adjustments, monitoring, trouble shooting and documenting/reporting requirements. Without physician oversight, identifies situations in which the pumping process is either damaging the kidney and/or is not achieving the intended goals. Utilizes critical thinking in decisions to mechanically and/or pharmaceutically intervene as necessary for improved preservation effectiveness and recognizes when medical director input is needed to ensure organ viability.

o Administers medications to preservation solutions and pumped kidneys according to established protocols and procedures.

o Understands the clinical requirements for kidney biopsies and ensures specimens are obtained in relevant circumstances. Uses independent judgement to determine the sites on the donor kidney that should be biopsied and uses proper clinical technique to obtain the biopsy specimen without causing damage to innate renal structures and precluding the ability to transplant. Preserves, labels and transports biopsy specimens for preparation. Scans and uploads biopsy slides to telepathology contractor.

o Facilitates the use of extra-corporeal, non-renal organ perfusion technologies.

o Independent of surgeon oversight, procures organs and/or tissues for research according to protocols established by requesting investigators.

• Ensures the safe and timely acquisition of organs from deceased donors outside of OurLegacy donation service area that have been prioritized for transplant candidates at AdventHealth Transplant Institute

o Provides logistical management (ground and/or air transportation) for surgical teams of all organ types (heart, lung, kidney? and liver) to and from the hospital at which the donor surgery will occur

o Is capable and willing to travel in non-commercial aircraft.

o Evaluates equipment and personnel capabilities available at the distant donor hospital. Rapidly creates contingency plans when needed supplies/equipment are not available or conducive to organ procurement.

o First assists Advent Health Transplant surgeons during the procurement surgery

o Exercises professional judgement that ensures timely communication and coordination between the donor surgical team and the recipient surgical team to minimize organ cold ischemic time

o Conducts organ preservation procedures for hearts, lungs and/or livers to ensure post-transplantation viability

Packages and labels organs for transport

• Provides perfusion and intraoperative support to transplant centers departing the OurLegacy service area for liver, heart, lung recoveries when appropriate.

• Builds rapid rapport with donor hospital operating room staff and with physicians/preservation personnel from non-AdventHealth transplant programs to ensure all participating parties are working in coordination and in support of the organ procurement process. Recognizes when interpersonal conflicts arise during surgical procedures and diplomatically intervenes to de-escalate tensions.

• Oversees the set-up of all necessary OR equipment and instrumentation required for organ recovery.

• Accurately distributes organs and/or specimens to transplant centers, labs and/or airports.

• Completes all required documentation within required timeframes.

o Documents all activities in the electronic donor record.

o Assures all documented information conforms to regulatory requirements and accurately reflects the condition of the preserved organ

o Uploads pulsatile preservation data to the national organ sharing IT platform to facilitate timely communication with potential accepting centers

• Manages and maintains all clinical organ inventory and supplies and reorders as needed to maintain par levels.

• Oversees and ensures that the organ preservation room is prepared in advance for procedures and cleaned at the end of each procedure in accordance with policy requirements.

• Archives required blood specimens in conformance with regulatory requirements.

• Completes all organ procurement cases once they have been started. This may require work beyond the end of regularly scheduled work time.

• Operates independent of direct managerial oversight in the field and has the autonomy to resolve complex surgical procurement and regulatory concerns.

• Recognizes when it is necessary to seek the advice of the Medical Director, or their designee, when circumstances require.

• Performs other duties as assigned or needed to ensure timely acquisition of donor organs, preservation of the organs, and timely delivery for transplantation.

Administrative Functions:

• Serves as an ambassador for the importance of organ donation and the respect that should be accorded to organ donors and their families.

• .

• Complies with all applicable CMS and OPTN policies and requirements.

• Maintains competencies and demonstrates comprehension of learned tasks on an annual basis.

• Serves as a consultant to local transplant centers by providing intraoperative support and education with living donor nephrectomies.

• Assists Hospital Services department in providing education and support to operating room staff at all service area hospitals.

• Participates in cross organizational activities to improve OurLegacy performance .

Qualifications

The expertise and experiences you'll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

2 years experience

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

• Certified Surgical Technologist certification

• Valid Florida driver’s license

• Graduate of an accredited Surgical Technology program or 2 years previous experience in a clinical healthcare role or current Certified Transplant Preservationist (TP) or Certified Procurement Transplant Coordinator (CPTC) certification
