Summary:

Processes requests for completion of patient-related forms/documents in a timely and accurate manner. Forms/documents consist of (but are not limited to) medical information requests from MetroHealth patients & clinical facilities, paperwork to be completed by clinical or administrative representatives of MetroHealth, and other documentation. Completes forms and submits for review by clinician as warranted. Ensures forms and documents are gathered and appropriately released to the patient or appropriate person(s) or facilities). Tracks the process of forms completion to ensure timely turn-around and patient satisfaction. Performs a variety of clerical duties in support of the clinical departments and is responsible for processing and releasing confidential medical record information as required. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.

Qualifications:

Required: High School graduate or passage of a high school equivalency exam. Three years of relevant experience in a physician office, clinic or hospital business office, or health care customer service setting. Computer literate with proficiency in Microsoft Word and Excel. Familiar with medical record documents and electronic health record. Excellent customer service and communication skills. Detail-oriented with strong problem-solving skills. Ability to work independently without close supervision in a team environment. Reliable transportation to travel to sites as needed.

Preferred: Some college level course work to include courses in a health care related field Medical Office or Medical Records experience. Experience with Epic. Experience performing forms completion or release of medical record information. Knowledge of medical terminology. Fluency in writing, speaking, and interpreting both English and another designated foreign language.

Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
