Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking a full-time Outpatient Counselor to monitor and track clients' progress of assigned caseload, and to provide individual and group outpatient counseling services for an assigned caseload of clients.

Additional responsibilities include the following: Provide individual, group and/or family therapy according to each individual's treatment plan. Complete and maintain all chart documentation according to insurance and licensing regulations including progress notes, treatment plans, concurrent reviews within established timeframes. Assure each client has input into developing an up-to-date treatment plan; modify plans to adapt to special needs and/or service delivery gaps. Establish an on-going dialogue with other members of the rehabilitation/treatment team, including residential staff, casemanagers, vocational/educational instructors, consultants, members of the medical profession and family members, as needed, to provide continuous supports; schedule planning and linking conferences as needed. Serve as an advocate for clients to assume a proactive role in their rehabilitative process. May provide intake/assessments for individuals referred to Mental Health/Substance Abuse Services as needed.

Job Qualifications:

M.A./M.S./M.Ed. in the Social Sciences, Healthcare, or a related field; at least 2 years experience in the MH/MR field. Prior experience  in an outpatient setting helpful. Good communication skills. Good writing (documentation) skills. Ability to work independently and as part of a team. Knowledge of mental health systems and supports.
