Job Description/Essential Functions:

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,5000 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting. Currently, we are seeking full time Program Specialist

The essential functions are to achieve and to maintain compliance with regulations within PA Chapters *5310; *6400 and *6500 that reference the work/service responsibilities of Program Specialists.

  • To provide training (1) to participants upon enrollment, and annually, rights of the individual and civil rights, complaint process and grievance process, and (2) to residential staff on provisions and updates to the Individual Support Plan (ISP).

  • To provide on-going evaluation to monitor the necessary level of services required to promote an individual's growth.

Job Qualifications:

This position requires a M.A. Degree in Human Services/Social Sciences/Healthcare or a related field and at least 1 year of experience in the MR field; or a B.A. Degree and at least 2 years of experience in the MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.

  • Microsoft office skills, spreadsheets and ability to type at least 55 wpm.

  • Excellent problem solving, oral and writing skills.

  • Good verbal, written and analytical skills.

  • Possession and retention of a valid driver's license with an acceptable driving record.

  • Very good communication skills; prior experience in writing service goals/outcomes, required.

  • Ability to work independently and detail oriented.

  • Ability to perform work to achieve and to maintain compliance with regulations within PA Chapters 5310, 6400 and 6500 (as applicable to position) which reference the work/service responsibilities of Program Specialists.

  • Ability to arrange, schedule and coordinate all participant services relative to the Agency's mission, and to participant programming goals for both internal and external service provision.

  • Ability to request, coordinate an provide written and oral assessment reports to participants, coordinating staff, authorizing agencies and significant others, as required. Write the monthly and quarterly accountability summaries and progress reports as mandated by the local jurisdiction and respective regulations.
