Position Summary
The Program Specialist position holds the responsibility of supporting health, safety, and growth of the individuals in the program; including medical requirements, programmatic concerns, incident reporting and individual finances. A commitment to the RHD values should be demonstrated as job duties are performed.
Schedule, chair and document ISP meetings and consumer reviews along with SC. Monitor annual ISP progress for all consumers, along with SC's and other members of the consumer's team. Ensure all ISP's are accurate and person-centered
Write instructional plans for the implementation of goals, objectives, and outcomes to implement consumer long and short-term goals and train all staff in the implementation of ISPs annually
Provide staff training as it relates to service delivery requirements and expectations as outlines in each consumers ISP.
Organize, maintain and update consumer files in accordance with the DPW 6400 regulations
Audit and monitor files at the office and sites
Participate and coordinate licensing preparations and provider monitoring paperwork
Complete and review annual assessments and progress summaries for all consumers, all monthly and quarterly progress notes and lifetime medical histories
Ensure that all consumers' medical, dental, psychiatric, and psychological services are scheduled in accordance with 6400 regulations and doctor recommendations
Attend consumer medical and psychiatric appointments in the absence of Site Supervisor
Ensure that all service delivery forms, individual goal sheets and data sheets and other forms as requires by the regulations, RHD, Division or program are completed in a timely manner and accurately reflect the consumer's individual needs.
Required Experience
2 Years Experience working with Individuals with Intellectual Disabilities
Required Education
Bachelor's Degree from an accredited college or university.
Required Qualifications
Valid Drivers License
