GENERAL DESCRIPTION OF POSITION

This position is part of the medical clinical team. This position is responsible for providing primary care services, including assessing and managing patients in skilled and intermediate nursing facilities following established standards of practice. This position shall be authorized to make independent decisions about advanced practice clinical needs of families, patients and medical decisions based on authorization for collaborative practice with one or more physicians.

Requirements

  1. On-site visits to Long-Term Care (LTC) facilities to develop and manage the medical plan of care for patients in coordination with the patients primary provider, if different than the APP, and LTC facilities, physicians.
  2. Provides complete, comprehensive outpatient care for patients including health examination and treatment of medical conditions, to include follow-up of problems detected.
  3. Responsible for managing acute and chronic illness, and routine care through care plans that are coordinated, when appropriate, with the patient\'s behavioral health provider
  4. Responsible for making appropriate referrals to internal disciplines of CHC/SEK and/or external resources/agencies for additional services required.
  5. Prescribes medications for patients for treatment of conditions, as indicated.
  6. Provide clinical consultation to other providers within CHC/SEK as needed.
  7. Responsible for accurate, timely, and clear medical record documentation.
  8. Observe clinic protocols and guidelines and scope of practice as determined by professional regulatory agencies.
  9. Provide appropriate follow-up with clinical support staff, including telephone/written correspondence (e.g., inform patients of test results).
  10. Provides clear written and oral communication to patients/guardians, and employees.
  11. Participate in patient care plan meetings and provide education as needed to facilities to enhance patient care.
  12. Participates in an on-call system for after-hours care for CHC/SEK inpatient population, as assigned.
  13. Participates in quality improvement activities.
  14. Ability to weigh the relative costs and benefits of a potential action.
  15. Supervise healthcare students as assigned.
  16. Average number of billable visits to be 10-14 daily.
