Are you a licensed LCPC or LCSW looking for flexibility, benefits, and a great team to work with? Recently merged with Heritage Health Center in Wyoming, One Health is looking for a Wyoming-licensed Behavioral Health Care Provider (LCSW/LCPC) to join our integrated health care team in Powell. This is an on-site position with flexibility of hours, either part-time or full-time. To be eligible for medical benefits, you must be employed at least 20 hours/week. ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

Evaluates and treats patients/clients (including walk-ins) within licensed scope of practice; evaluates and treats mental illness and substance abuse, treats or appropriately refers patients, for specialty evaluation and treatment, provides preventive health/lifestyle advice and case management, and provides consultation to other staff.
Participates in multi-disciplinary, team-based care of patients/clients.
Provides consultation to other members of primary care and behavioral health care teams.
Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.
Participates in the student and/or other clinical discipline training activities at the center.
Participates in the formation and evaluation of applicable protocols.
Maintains continuity of care when health center patients utilize other health care facilities, i.e. referrals, emergency room services, outpatient departments, specialty clinics, and inpatient services.
Provides documentation according to health center procedures and follows up on results.
Participates in staff meetings, including quality assurance and behavioral health audit activities in keeping with health center procedures.
Participates in on-site and off-site outreach and continuing education programs and ensures training to maintain license and to meet the needs of the population.
Adheres to all One Health Policies and Procedures.
Participates in performance optimization and risk management activities as required.
Cooperates and abides by all elements of One Health’s Compliance Program.
Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.
Performs other duties as assigned or required with or without prior notification.
MINIMUM JOB QUALIFICATIONS:
Master’s Social Work (MSW) and licensed in the State of Wyoming as a LCSW, OR
Master’s in Counseling and licensed in the State of Wyoming as a LCPC
Valid driver’s license
Two (2) years community health experience

WORKING CONDITIONS AND ENVIRONMENT:

Work indoors in heat-controlled environment 95% of the time.
Moderate noise level consistent with an office environment.
Typically, 40-hour workweek.
Some evening and weekend hours as needed.
Periodic travel to other One Health sites and within Wyoming/Montana as well as regional and national meetings.
