Specific Responsibilities:

This is a Health Services Representative position assigned to the Northeast Health Center providing medical service support in the Family Planning/ IDC/ Maternity/GYN. Supports the department and/or organization to provide health care.This is a bilingual position fluent both in English/Spanish.Work is performed under the supervision of a Senior Community Health Nurse Supervisor.This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. This position will be cross trained to assist in taking patient histories in STD clinics as needed.

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures. This position have access to the following system(s): Health Management System (HMS) for entering Employee Activity Report (EAR's), HMS for the purpose of reviewing patient medical records, scheduling patient follow up appointments as needed and print out patient schedules for clinic; Electronic Health Record (EHR) for the purpose of documenting and reviewing patient medical record, printing of progress notes and entering data; Surveillance Tools & Reporting System (STARS) to view, update and release SID results. May view, enter, update and print data from the HMS- Federally Qualified Health Center (FQHC) Assessment Module. This position may review, update and release "CONFIDENTIAL" medical information forcontinuity of care using the HMS data system and view, update and release STD results from STARTS surveillance data base.

Assist the nursing staff in conducting patient interviews for the Maternity intake process, identifying lab tests on the Prenatal Orders with available results to prevent duplication and accurately documenting information on appropriate Florida Health/CHD Prenatal Record DH 3142, Prenatal Genetic Screening PBC 281.0, Tetra or Triple Screen Questionnaire, CHD "Peace of Mind" Consent, TB Screening and Evaluation PBC 254.0, Help Your Baby Have A Healthy Start In Life DH 3134S, and Progress Note DH 3056 form. Ensures patients receive a physical appointment at the time of intake appointment. Assist with health education of the clients. Obtain and document patients histories in Maternity, Family Planning, lDC and GYN forms as instructed. Assist in taking patient histories in STD clinics as needed, including but not limited viewing, updating and release results from the STARS data surveillance base.

Determine patient demographic information as needed and contact patients to verify insurance coverage. Assist management with the UDS data

Collection of "Characteristics of Prenatal Care Patients·, Trimester of Entry into Prenatal Care· and "Deliveries and Birth Weights, Screen and refer eligible maternity clients to the Centering Program. Connect post-partum clients to the appropriate medical care as needed for continuum of care. Schedule and coordinate Gestational Diabetes classes with nutritionist. Ensure clients have the appropriate paper-work and appointment(s) with CHD and /or community partner(s).

Assist as needed with daily Family Planning/ IDC/Maternity/GYN flow, assign patients to the Health Support Aides for vital signs measurements, and assist the Health Support team as needed. Work directly with the Triage Nurse, place patients in exam rooms, honoring appointment visits over walk-in visits. Direct Providers to the next patient by using the door-flag-system and chaperone provider during exam as needed. Ensure Nurses follow the same flow process as the Providers to ensure patients are seen in the correct order. Coordinate quick switch between Providers and Nurses in the examination rooms, including Immunizations for the patient's convenience, including but not limited to, ensuring labs are in medical record, making specialty appointments, ensuring all referral related paperwork is complete. Placed specialty report in the patient medical records, retrieve same day lab reports for the Providers to review when clients return for follow-up appointments. Assist Health Support and Nursing staff making sure rooms are cleaned, ready and adequately equipped. Responsible for keeping appropriate patient check in clinic logs, daily patient census and services at the Nursing station for Leadership updated, about current patient needs and clinic operations. Inform administration in a timely manner about identified bottle necks in clinic flow, to ensure timely decisions and appropriate actions can be taken. Closely monitor clinic supplies and equipment in the Family Planning/ IDC I Maternity/GYN clinic and communicated timely with administration regarding items can be ordered.

Assists the Nursing staff in tracking broken appointments, both clinic and referrals, and tracking patients requiring follow-up in accordance with FDOH Guidelines. Participates in staff meetings, planning sessions and Quality Improvement Programs. Attends staff meetings and in-service programs as assigned.

Required Knowledge, Skills, and Abilities:

  • Basic to intermediate computer skits and knowledge of computer programs (Microsoft Word, E-mail and internet)
