    Salary:$25-$33/HR

    As a Licensed Practical Nurse, you will implement and coordinate the delivery of care in collaboration with physicians and care associates. You will be responsible for the high level of care our residents are accustomed to, including making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents’ overall condition and behavior. Above all, you will exhibit a passion for serving seniors and contributing to the positive environment our residents are proud to call home.

    • Must possess the ability to communicate effectively, including written and verbal skills; work well, both individually and as part of a team, through respect, collaboration, and dedication; and successfully interact with a diverse culture base

    • The successful candidate must prioritize and organize work effectively and efficiently and demonstrate a positive attitude, high integrity, and ethical awareness.

    • Responsible for new admissions and hospital returns, ensuring care plans are updated, implemented and maintained.

    • Utilize the nursing process (assessment, planning, implementation, and evaluation) for the best care of our residents

    • Provide health and wellness checks to our residents on a regular basis

    • Assist in the development and update of individual service plans for each resident

    • Maintain ongoing communication with residents, their families, and their physicians regarding their needs

    • Supervises and assists with residents’ activities of daily living.

    • Assists in coordinating nursing service activities with those of other departments.

    • Observes resident, records significant conditions, vital signs, and reactions, notifies supervisor or physician of any change in resident’s condition, and updates charts to reflect this.

    • NY State license must be in good standing
