Provide assistance and instruction in activities necessary to promote and maintain independence for individuals with intellectual/developmental disabilities who chose to Self-Direct their services. The Self Directed Program provides as needed care to individuals as assigned, to meet their daily needs and enable them to function at the highest possible level.

  1. Assist individual with specific facilitative outcomes as described in each individualized Self-Directed Plan. These duties may include personal care, community integration, money management, advocacy, employment needs and respite.

  2. Continually seeks opportunities to meet individual’s psychosocial needs through both care planned and

    Spontaneous actions.

  3. Provide reliable, safe, high quality services as directed, in a thorough and caring manner.

  4. Implement a person centered approach to services.

  5. Follows universal precautions, proper infection control, sanitation and safety standards of practice in all work and activities.

  6. Completes paper time sheet/invoice and web based time keeping in a timely manner in accordance with payroll processing.

  7. Communicate with supervisor and Circle of Support about individual’s habilitation plans that may need revisions or updates.

  8. Communicate effectively with individuals with intellectual/developmental disabilities, the Circle of Support, Broker, Fiscal Intermediary Coordinator, service coordinator, supervisor, interagency professionals, community professionals and families.

  9. Consistently works with all team members, led by individual and family preference to meet the needs and desires of the individual served.

  10. Respect the individual’s dignity, privacy, property, religion, and culture.

  11. Document and maintain accurate records of contacts with individuals.

  12. Provide appropriate interventions to all crisis situations. Provide immediate notification to appropriate person of any allegation of abuse or neglect.

  13. Attend mandatory training.

  14. Provide transportation, as needed, to individuals and follow safety procedures regarding transportation.

  15. Maintain flexibility in schedule based on needs of individuals assigned.

  16. Be dependable – to arrive on time and be ready to go to work.

Other duties as assigned by individual being served or CCCS supervisor, if driving is a responsibility

QUALIFICATIONS :

EDUCATION :

High School Diploma or equivalent.

EXPERIENCE :

One year practical experience working with individuals with intellectual/developmental disabilities preferred. A combination of experience and education may be substituted at the discretion of the Executive Director.
