Job Summary

The University of Utah Department of Pediatrics is excited to announce an immediate opening for a School Based Health Care Coordinator at our West High School Clinic ( WHSC ) location. We are seeking an enthusiastic, creative and hard-working individual who, in collaboration with the Medical Director, Nursing Supervisor and Medical Assistants of West High School Clinic, will provide care coordination support to multiple specialties in an integrated multidisciplinary collaborative care clinic. The WHSC offers multidisciplinary services, including primary care pediatrics, developmental pediatrics, counseling, psychiatry, and addiction psychiatry services The School Based Health Care Coordinator plays a key role in the clinic by providing complex care coordination, hands-on as well as facilitating the provision of care coordination activities by members of the clinic team. The School Based Health Care Coordinator also works with the clinic team in building systems, (re)designing processes, leveraging health information technology and collaborating across the clinic to improve the quality, equity and outcomes of care.

Responsibilities

  • Care Coordination: Provides follow-up and support as needed with patient and patient families at the request of the physician and care team. Assist patients in navigating the healthcare system. Tasks may include but are not limited to: access to health insurance, Medicaid applications and eligibility, explanation of insurance benefits, resources for under or uninsured patients, and scheduling of follow-up visits or referrals. Responsible for tracking clinic referrals and auditing referrals periodically to verify appropriate follow-up.

  • Under the direction of the physician and care team, may assist in identifying and/or drafting plans of care for various medical and behavioral conditions, drafting templates for better treatment of conditions, and compiling educational handouts for patient and patient families regarding various medical conditions. Provide support with school accommodations such as Individualized Education Programs (IEPs).

  • Support practice in defining, implementing, and continuously improving clinic by utilizing tools and resources such as electronic medical records (EMR’s), patient portals, patient registries, assessment and screening tools, clinic guidelines, care and action plans, referral tracking logs, and patient education resources.

  • Utilize project management tools and skills to: Define and manage project scope, task requirements, project plans, status tracking/reporting, implementation, and sustainability strategies/plans. Define and coordinate appropriate resources to accomplish tasks and support implementation and sustainability.

  • Quality Improvement: Participate in the use of quality improvement techniques and tools to:

  • Identify relevant improvement opportunities

  • Design and implement improvements Measure their impact

  • Hold the gains

  • Seek opportunities to continuously improve

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient’s status and provide care as described in the department’s policies and procedures manual.

Minimum Qualifications

Bachelor‘s degree or equivalency (one year of education can be substituted for two years of related work experience) in public health, health education, health promotion, management, behavior science, social science, social work or nursing with 2 years relevant work experience or Medical Assistant with State of Utah, National Certification, or equivalency and 5 years of clinical experience.

Demonstrated ability to lead as well as function effectively as a member of a team and successfully promote change. Strong analytical, organization and leadership skills. PC/Windows literate with working knowledge of the following Microsoft applications: Outlook, Word and Excel. Demonstrated human relation and effective communication skills.

This is a patient-sensitve position.

Preferred Experience

Working knowledge of quality improvement and project management methodologies, tools and techniques and principles of change management. Demonstrated ability to effectively facilitate and mentor project teams preferably using QI methods (i.e., PDSA ). Familiarity with electronic medical record ( EMR ) applications.
