Hourly Pay Range (Minimum - Midpoint - Maximum):

$126.966 - $163.827 - $200.688

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do

The Chief Medical Officer and Director Hospice and Palliative Care serves as the physician leader of the service line. This position serves as the Hospice Medical Director and is responsible for the medical component of the patient care program in accordance with the Hospice Medicare Condition of Participation 418.102. Provides oversight of the 24/7 hospice physician provider team including clinical expertise and guidance.

Provides leadership and accountability in achieving system vision, financial and operational goals/targets; develops plans and implements initiatives to support quality improvement, clinical outcomes, patient safety, and clinical efficiencies.

Required Qualifications

  • Doctor of Medicine (MD)- Internal Medicine, Emergency Medicine, Family Medicine, Oncology/Hematology, Pulmonary, or Nephrology.

  • OR Doctor of Osteopathic Medicine (DO)- Internal Medicine, Emergency Medicine, Family Medicine, Oncology/Hematology, Pulmonary, or Nephrology.

  • 8 years' experience in clinical specialty.

  • California Physicians and Surgeons License - Medical Board of CA -REQUIRED

  • Driver's License - CA Department of Motor Vehicles -REQUIRED

Other Qualification Requirements

  • Board certification Palliative and Hospice Medicine.

  • Board certification - licensed to practice medicine in the State of California.

  • Unrestricted CA MD/DO license required.

  • Utilizes reliable transportation and possesses adequate personal insurance coverage.

  • Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.

Essential Functions

  • Quality Assumes the responsibility for the Hospice's patient care program's medical component.Provides leadership oversight to palliative and hospice physicians to ensure evidence-based quality care.Regularly reviews and analyzes quality related data and trends and works with team members to develop and implement performance improvement plans as required.Leads clinical effectiveness efforts including the development of clinical protocols, benchmarking, and best practice identification.Provides an environment of clinical excellence and cost improvement that is data driven. Develops systems to review utilization of resources and objectively measure outcomes of care in various care settings, e.g. inpatient and community.Monitors physician practice patterns and presents data analysis and interpretation to physicians and committees/workgroups to improve care practice and processes.Participates in development and implementation of systems and processes that ensure the highest quality of patient care, safety, and evidence-based care across care settings.Serves as a liaison between hospital, medical groups, and hospice medical staff.Participates in committees and workgroups to advance the integration of palliative and end of life care services.Facilitates, intervenes, and moderates physician-physician, physician-patient, and physician-staff concerns in collaboration with appropriate Medical Staff and management team members.Maintains working knowledge of applicable national, state, and local laws and regulations affecting hospice and palliative care including Joint Commission, Title 22, and CMS Hospice Medicare Conditions of Participation.Participates in meetings to provide input into the strategic and operational discussions and deliberations for issues pertaining to hospice and palliative care.Support and establishes a system that assures the provision of necessary patient care services, timely and at the appropriate level of careRepresents Hospice and Palliative Care in a professional, competent manner in the community, state, region, and national meetings.Provides leadership and accountability in achieving vision, financial and operations goals and targets for hospice and palliative care.Participates in CME presentations pertaining to palliative and end of life care topics.

  • Service/People Collaborates with the leadership team to ensure that each patient/family perceives their overall experience as exceptional, The Sharp Experience.Works the system and hospice medical staff to develop, implement and/or support opportunities and processes to improve the patient/customer experience.Establishes and maintains a collaborative working relationship with the medical, nursing, and administrative staff in hospice and the system.Participates in decision making that affects the provision of medical services for hospice patients and their families.Actively promotes the professional growth and future development of medical leadership through educational outreach, professional organizations, and affiliations.

  • Finance Supports the financial growth of the organization.Participates in identifying operational cost efficiencies and provides recommendations for cost improvement initiatives.

  • Growth Works collaboratively with system physician leaders for growth opportunities to expand and further integrate the hospice and palliative service lines across the continuum of care.Participates in developing and implementing innovative programs/services and adapts current programs/services to meet organizational needs across the continuum.Works closely with the Business Development team, hospice administration and system leaders to leverage opportunities for program growth of hospice services.Works closely with the Office of Transformation and hospital committees/departments to leverage palliative and end of life care services throughout the continuum.Focuses on increasing market share of hospice admission growth.

  • Community Represents Sharp HealthCare at appropriate professional organizations.Participates in community outreach, education, and other activities to promote Sharp HealthCare including philanthropy.

  • Hospice Medical Director Duties Ensures 24/7 hospice and palliative care staff for clinical guidance and patient care needs.Participates as a member of the CORE Interdisciplinary Team. Participates in review of policies and procedures governing daily operations regarding patient/family care.Reviews and documents the clinical information for assigned hospice patients and provides written certification that it is anticipated that the patient's life expectancy is 6 months or less if the illness runs its normal course.(1) The primary terminal condition(2) Identification of all related and unrelated diagnoses(3) Current subjective and objective medical findings(4) Identification of medication regimen and determines which medications are hospicerelated/covered and which are not.(5) Reviews the recommended treatment interventions per attending MD and provides input into appropriateness related to terminal illness and hospice plan of care.Works closely with the Quality/Utilization Review Department to correctly identify the most appropriate ICD 10 codes.Before the recertification period for each patient, as described in § 418.21(a), reviews the patient's clinical information and determines continued eligibility in collaboration with attending MD. Documents according to Medicare requirements for recertification.Participates in the establishment of the plan of care in conjunction with the attending physician and Interdisciplinary Team prior to providing care.In conjunction with the attending physician and Interdisciplinary Team review, establish the plan of care at intervals no greater than 15 days.* Reviews and signs C2 (Controlled Drugs) prescriptions and returns to Pharmacy provider as required by law.* Reviews and signs Death Certificates ensuring accuracy of information and returns to mortuary.* Is available to facilitate communication between the medical staff and hospice staff as needed.* Makes home visits with hospice staff as warranted for clinical guidance and changes in the Hospice Plan of Care.* Assumes attending physician role for patients on General In-Patient level of Care in the hospice homes as assigned.* Provides hospice nurse practitioner supervision to include availability on site or by telephone. No physician will supervise more than four (4) nurse practitioners at any one time per standardized procedures for* Participates in educating hospice staff through formal and informal in-services.* Participates in Case Conference meetings to review active cases as requested.* Documents in the Electronic Health Record per agency protocol.* Performs other duties as requested.* Performs Face-to-face encounter visits in accordance with Medicare requirements and Hospice policy.

  • Safety Promotes the Sharp HealthCare Patient Safety program and achieves improvement in safety reporting and reductions in safety events including Culture of Safety survey response rates and use of the survey results in improvement efforts.
