The Panel Navigator is a Classified Staff Position: Patient Services Specialist 2 (NE H SEIU 1199NW UWMC Northwest Service and Maintenance) UW MEDICINE: UW Medicine’s mission is to improve the health of the public by advancing medical knowledge, providing outstanding primary and specialty care to the people of the region, and preparing tomorrow’s physicians, scientists, and other health professionals. UW Medicine owns, operates, or is affiliated with Harborview Medical Center, Northwest Hospital & Medical Center, Valley Medical Center, and UW Medical Center, a network of UW Medicine Primary Care, UW Physicians, UW School of Medicine, UW School of Nursing, Airlift Northwest, and other owned, operated or affiliated entities as appropriate. In addition, UW Medicine is clinically integrated with the Fred Hutchinson Cancer Care Center. POSITION: The Colorectal Cancer (CRC) Screening Panel Navigator is responsible for outreach and care coordination services as they relate to the Fred Hutch / UW Medicine CRC Screening Program, which is focused on closing care gaps in CRC prevention for UW Medicine patients. The CRC Screening Panel Navigator is a part of a central outreach team and serves as point-of-contact for assigned patient populations that are due for CRC screening. Assignments will be based on defined sub-population groups where there are gaps in care, aligned with the overarching strategy and priorities of the CRC Screening Program. They will provide patient-centered concierge services to better coordinate patient care within the UW Medicine health system and partner/affiliate organizations. In communication with care teams, this individual will mitigate potential roadblocks and navigate patients for optimal care delivery, including providing support for planning, scheduling appointments and coordination of services/appointments. The CRC Screening Panel Navigator will perform work in support of the joint Fred Hutch / UW Medicine CRC Screening program and under the general direction of the Program Manager, Population Health, UW Medicine Primary Care. RESPONSIBILITIES: This position works closely with other roles in the Panel Navigator team and responsibilities include: • Patient outreach, care navigation and coordination • Appointment scheduling • Referral coordination • Medical record activities • Other program support activities REQUIREMENTS: High school graduation or equivalent AND Two years of Patient Services Specialist (PSS) experience OR Equivalent education/experience. ADDITIONAL POSITION-SPECIFIC REQUIREMENTS: • Excellent communication skills to include verbal and written comprehension/expression. • High competency in emotional intelligence skills – ability to navigate conversations with patients and families in the most compassionate manner to result in an outcome that is right for patient’s health and meets programs goals. • Demonstrates good judgment and common sense. • Skill in operating personal computer and use of Office 365 tools. • Demonstrated ability and willingness to maintain competency as required for job title and the unit/area(s) of assignment. • Demonstrated ability and willingness to exhibit behaviors consistent with standards and principles for service excellence, performance improvement and organizational values. DESIRED: • Bachelor's Degree in a medically related or public relations field AND two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology preferred. • Proven skill in working with medical providers and wellness coaches preferred.
