Overview:\ Quality Care Access is seeking a dedicated Behavioral Residential Clinician to coordinate and oversee the delivery of care from a diverse team of health care professionals, including nurses, social workers, therapists, and frontline workers. This role involves designing, implementing, and evaluating interprofessional collaborative care plans, developing behavior management plans, conducting functional assessments, and creating positive support plans to ensure the highest standards of service and care delivery.

Key Responsibilities:

Interprofessional Care Coordination:

  • Design and implement interprofessional delivery of care plans.
  • Develop, deliver, and evaluate interprofessional collaborative (IPC) plans.
  • Ensure optimal and safe care delivery through close monitoring and field formative supervision.
  • Instruct Direct Support Professionals (DSPs) and Certified Residential Medication Aides (CRMAs) on addressing care needs.
  • Determine appropriate clinicians to provide needed quality care.

Behavior Management:

  • Develop comprehensive behavior management plans in consultation with relevant professionals.
  • Conduct functional assessments to identify the root causes of challenging behaviors.
  • Create and implement positive support plans tailored to individual needs.
  • Monitor and adjust behavior management strategies to ensure effectiveness.

Documentation and Review:

  • Assure accurate and continuous data management through standardized clinical documentation.
  • Audit progress notes in Therap to ensure compliance and accuracy.
  • Review and organize quarterly formative reviews of care delivery.
  • Organize and conduct summative annual evaluations of care delivery.

Client Assessment and Intake:

  • Participate in the intake of potential individuals and assess their care needs.
  • Develop individualized care plans based on comprehensive assessments.

Performance Evaluation and Training:

  • Participate in employees\' performance evaluations.
  • Play a crucial role in the team assigned to develop a master continuous training plan.

Supervision:\ The Behavioral Residential Clinician will be supervised by the Director of Health Care & Quality Assurance.

Qualifications:

  • Masters degree in Social Work, Social and Behavioral Sciences, or Mental Health Counseling from an accredited university, with at least two years of experience in the field.
  • Alternatively, a Bachelor\'s degree in Social Work, Social and Behavioral Sciences, or Mental Health Counseling from an accredited university, with at least five years of experience in the field.

Licenses:

  • Must hold one of the following State of Maine licenses:
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Social & Behavioral Sciences
  • Licensed Mental Health Counselor

Skills and Requirements:

  • Must meet initial and ongoing educational requirements per the State of Maine.
  • Proficient in both spoken and written English.
  • Strong organizational skills and attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with a multidisciplinary team.
