Premise Health Care Coordinator (LPN) in Sparks, Maryland

We have an opening for a Full-time Care Coordinator (LPN) to work with our Employer Health Center located in Sparks, MD !

Summary: This Care Coordinator (LPN) will work full-time in our Employer Health Center based in Sparks, MD. The scheduled hours are Monday-Friday 8:30am-5:00pm. No nights, no weekends, no on-call!

Essential Functions:

  • Identifies, facilitates and secures access to needed medical and mental healthcare services and appointments, social service benefits and community resources for the client’s member population and dependents.

  • Coordinates assessment, treatment plans, and return to work processes with the employee, providers, and employer.

  • Assesses and evaluates cases and implements treatment and case management plans.

  • Counsels clients, assisting them to identify and resolve problems and make effective use of resources.

  • Develops an understanding of Epic's referral and scheduling capabilities, and available tools.

  • Develops an understanding of where member populations live and work, as well as high-value providers and facilities in key geographies.

  • Proactively builds relationships with community specialists, alongside other health center team members, to educate them on Premise and our expectations, facilitate streamlined referral access and information sharing.

  • Interacts with Premise providers to understand referral needs.

  • Interacts with members via multiple modalities (phone, email, in-person, video, etc) to identify appropriate referral target(s).

  • Works with community specialists and members to schedule appointments and ensure bi-directional notes transfer.

  • Follows up with member post-specialist visit, ensure member and specialist notes return to Premise, and coordinate ongoing care needs as appropriate.

  • Remains in close coordination with central Member Health Management team as we evolve our Member Health Management strategy, leveraging new tools and processes.

  • Maintains collaborative working relationships with other community providers to develop specialized services as required.

  • Provides communication to staff on specific cases.

  • Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required.

  • Participates in case conferences to ensure that each member’s care team is aligned with a shared plan of care and up-to-date information on each member.

  • Provides written status updates on a regular basis as requested and verifies documentation of all such activities in member chart.

  • Maintain a directory of provider services including medical, behavioral health, social support and other services as needed to support referral and coordination activities. Identify valuable resources to share with the rest of the team to the benefit of all members.

  • Complete an in-depth initial needs assessment; subsequently overseeing a member-centered plan to address at risk areas.

  • Monitoring of the plan addressing at risk areas with documented follow up.

  • Complete accurate and up to date documentation; conduct daily follow ups to address any issues, review the previous day’s activities, and to discuss any client issues and or/new insight regarding a member.

  • May be asked to obtain and record patient's vital signs and weight.

  • All other duties as assigned.

Basic Qualifications

EDUCATION:

  • Graduate of National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from two-year college or technical school.

  • Licensed as a Practical or Vocational Nurse

  • Current hands on Certification in Basic Life Support is required, Advanced Cardiac Life Support may also be required based on scope of contract services.

EXPERIENCE:

  • At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting.
