Job Title: Hospice RN Case Manager

Schedule: Monday through Friday, from 8:00a to 5:00p

Key Responsibilities:

As a RN Case Manager you will:

• Assess and evaluate the physical, psychosocial, and environmental needs of your assigned home hospice patients, using clinical records, on-site visits, and team consultations.

• Conduct initial and ongoing evaluations of each patient’s nursing needs during every visit.

• Communicate significant patient updates or changes in health to the appropriate clinical supervisor, physician, or team members involved in the patient’s care.

• Implement safety and care plans, offering guidance and instruction to patients and their families based on specific care needs.

• Ensure quality care is delivered by participating in the execution of individualized plans of care, focused on achieving positive patient outcomes.

• Recognize and respond to any changes in a patient’s condition, initiating appropriate actions when necessary.

• Confirm and follow the patient’s Plan of Care prior to every visit, providing care according to the physician's orders, clinical assessments, and best practices.

• Take on additional responsibilities and projects as assigned.

What you'll Need to Bring:

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

• Current State Licensee as a Registered Nurse RN

• 1 year of experience in a clinical care setting

• Experience in hospice or home health environment is preferred

• Nursing skills as defined by accepted nursing standards

• Ability to handle multiple priorities, documentation requirements, and deadlines

• Reliable transportation to perform job tasks
